Homebound/Home Instruction Services

Students on a 504 Accommodation
may receive Homebound services if identified as actually ill by their physician (DO/MD). The Parent/guardian must provide medical documentation that establishes eligibility station the nature of the health condition and possible length of time the student will need to be away from school. These services are arranged by contacting the school counselor.Each site will have a 504 committee that will address a 504 student’s need for services.

Students on an Individual Education Plan (IEP), already actively enrolled in Salina Public Schools at the time of their illness/injury/medical condition, and who are unable to attend classes due to a health condition, discipline issues, or programming issues, may be eligible for Home Instruction Services.  They will be served in the home or in another designed area by a certified teacher.

A request for Home Instruction (IEP students) may be made when a Dr. (MD/DO) has determined that a student will be absent due to accident/injury/ or medical condition.  The parent/guardian must provide medical documentation that establishes eligibility stating the nature of the health condition and the possible length of time the student will need to be away from school to recover form the illness/injury/medical condition.  These services are arranged by notifying the Counselor or the students file teacher.  The student’s IEP team must meet to place the student on Home Instruction and determine services to be provided.
